疾病原因
曲菌病由曲菌引起。曲菌是zui常见的污染杂菌,种类很多,在人类曲菌病中,zui常见的致病菌为烟曲菌。曲菌生长迅速,在潮湿霉烂的谷物、稻草或腐烂的枯树叶中繁殖很快。因此谷仓、土壤、空气中常有大量曲菌孢子,可引起原发性肺曲菌病。外界环境中的曲菌孢子主要通过呼吸道进入人体。吸入的曲菌孢子不一定在人体内繁殖引起疾病。多数情况下曲菌是一种条件致病菌,只在机体抵抗力降低的基础上致病。曲菌可在身体许多部位引起病变,如皮肤、耳、鼻腔、眼眶、心、脑、肾、呼吸道、消化道等,而以肺部病变为zui常见。曲菌在组织内可引起化脓,形成小脓肿。有时不化脓而发生组织坏死及出血,周围有多数中性粒细胞和单核细胞浸润。在小脓肿和坏死灶内有大量菌丝。曲菌并常侵入血管引起血栓形成,造成血管阻塞,可使组织缺血、坏死。慢性病灶有纤维组织增生,其中有大量淋巴细胞和单核细胞浸润,并杂有多数巨噬细胞和异物巨细胞。病灶内有大量菌丝。曲菌菌丝粗细均匀,直径约2~7μm,有隔,分支状,常成约45°的锐角分支,HE染色呈蓝紫色,PAS染色更为清晰,也可用镀银法显示。在陈旧的纤维肉芽肿性病灶内,曲菌菌丝可变形,粗细不匀,部分可膨大呈球状或小囊状。

症状体征编辑
注意肺部有无哮鸣音和湿啰音以及与革兰阴性菌败血症相似的体征,如弛张热、血压下降、继发肺炎和败血症表现,相继出现胃肠道、肾、肝、心、脾和中枢神经系统等脏器受累体征。
疾病病因编辑
注意有无长期应用广谱抗生素、糖皮质激素及抗肿瘤药物,有无内分泌紊乱或原发病因治疗而引起正常免疫应答低下情况,有无严重烧伤、心脏手术或慢性病(如肝病、抗原抗体、糖尿病、血液病等)。
